Facility Personnel

Heartland Kidney Network routinely mails and/or faxes important information to your facility staff such as newsletters, patient rosters, compliance reports, etc. In an effort to ensure that the appropriate staff receives that information we send out several times a year a list of personnel for your facility for you to update. If you would like to know who we have on file for your unit, or if you have had personnel changes that you would like to report now please fax them to 816-880-1775 attention Serena Timko and include that person's:

• Full name
• Credentials
• Phone and/or fax number if different than the facility's main numbers
• Email address if available
• Their role, such as the Roster Recipient, Unit Administrator, 2728 and 2746 Forms, Social Worker, RN, etc.

When we receive a CMS Certification letter for a brand new facility, we mail out a "New Facility" packet. Included in the packet is a blank form to list all of your personnel. The New Facility Personnel form should be faxed to Serena Timko at 816-880-1775.
