From the FDA Baxter Healthcare has released this urgent voluntary recall notice for heparin that may cause the following patient reactions:
Baxter International Inc. announced today (2/28/08) that the company is proceeding with the voluntary recall of all remaining lots and doses of its heparin sodium injection multi-dose, single-dose vials and HEP-LOCK heparin flush products.
The company initially recalled nine lots of heparin sodium injection multi-dose vials on January 17, 2008 as a precautionary measure due to a higher than usual number of reports of adverse patient reactions involving the product and suspended production earlier this month.

The 2008 Heartland Kidney Conference and Annual Business meeting will be held at the Westin Crown Center Hotel in Kansas City from Wednesday January 9th to Friday January 11th, 2008.
CROWN Web is coming to your unit in 17 months (February 2, 2009). On this date all units will enter patient data electronically, including the monthly Patient Activity Reports and Facility Personnel Updates as well as Forms. Volunteer testers from the provider community have been recruited to preview, test and comment on the development of CROWN Web.
Quarterly patient rosters will be mailed out April 16th, and are due back with your changes by May 4th. The Facility Personnel Updates will be mailed on April 20th, and are due back with any changes by May 11th.
The Annual Facility Surveys were mailed out to all units today, and are due back in our office no later than February 16th. Please be sure to send the Fax Back to us upon receipt of the survey.
Today is the cut-off date for late registration for our NCC-2007 Annual Meeting January 11-12, 2007 at the Westin Crown Center Hotel.
On December 15th a sample of the 2744 Annual Survey was mailed to all facilities. This sample will help you to prepare for the real survey that will be mailed to you in January.
Nominations for the PCT (Patient Care Technician) awards to be given at the January NCC 2007 Annual meeting are due to the Network office by November 1st, 2006.

CMS funded Network 8, Inc. to conduct a special outpatient medications project from July 2004 – June 2005 in order to evaluate a number of safety and coverage issues related to drugs that are available to the CKD Stage 5 population. Emphasis was placed on medications to be avoided as well as those that should always be available.
