Privacy Policy
The privacy of our customers is important to Heartland Kidney Network. Therefore, the following is our Internet privacy policy.
- The network will make every attempt to protect the personal information that you share with us.
- Due to HIPAA and Privacy Act regulations, we cannot accept PHI (Personal Health Information) by email as it is not secure against interception.
- We are further required to report other healthcare entities sending us PHI via email to Medicare as a HIPAA violation.
- We recommend that if your communication is very sensitive, or includes personal information such as your Social Security number or medical history, that you send it by postal mail instead. Faxing is acceptable and our fax numbers are listed above.
- The information you send may be shared with other Network, CMS, or Medicare representatives if applicable and with your permission. It will not be shared with any other third parties.
- Your information will be used for such purposes as described at the point of collection.
